I have consulted the regulations of LegsApparel Academy indicated above, in the website www.legsapparel.com and accept that my private information is only used for the application acceptance process.

If the student is below the age of 18, parent/guardian signature is needed:

Declaration of Applicant: I certify that the particulars furnished are true and complete in all aspects, and no information has been withheld. I understand that falsifying documents or information on this application may result in penalties up to and including immediate/ permanent dismissal from the Academy. I understand that misrepresentation, falsification of documents, and/or the withholding of requested information in regard to this application are serious offences that may result in prosecution. I agree to the terms and conditions of the academy stated on the Legsapparel website.

Upon receipt of completed application and acceptance into the academy, students must provide the appropriate enrollment fee. An official enrollment agreement will be issued and your name added to the list for the program/course of your choice.

Note: Course offerings, schedules, tuition fees, facilities and faculty are subject to change without notice. We also reserve the right to cancel or postpone classes due to inadequate enrollment.
